Summary:

Transitions of Care and Rehospitalizations Gaps in care coordination and patient engagement contribute to hospital readmissions and poor outcomes.

Although hospital unplanned readmission rates have dropped from 21.5% to 17.5% since 2007, there is additional opportunity to improve care coordination between sites of care and healthcare providers.  Through both improved care coordination, communication and accountability between health care providers and improved patient engagement healthcare professionals will learn strategies that lead to improved patient experience and outcomes.

 

Learning Objectives for this presentation:

Participants will gain an understanding of:

-how rehospitalizations impact patient outcomes and health care costs -the importance of inter-disciplinary collaboration in preventing re-hospitalizations -practical easy to implement tips to improve transitions of care

 

Speaker: 

Brad Miner, Sr. National Director of Client Services and Clinical Programming for Rehabcare. Brad has spent his entire career in healthcare.  He has worked with LTAC and Inpatient Rehab Hospitals previous to joining Kindred Healthcare and Rehabcare.  He is currently focused on partnering with Rehabcare Hospital, Skilled and Home Health clients on physician relations, preferred provider, ACO, Bundling and Program Development opportunities across the country.
