1. “TPOPP/POLST: A Better Means of Expressing Advance Directives”

Ensuring persons express their advance directives and correctly translate them into a code status is a fundamental right in health care. TPOPP/POLST is an excellent tool for frail and elderly persons that allows a fuller expression of health care wishes and expectations compared to the Missouri Out of Hospital DNR (MO OHDNR) form, and that now has ambulance indemnity similar to the MO OHDNR form. This session will discuss how TPOPP/POLST is used, ideal candidates, translation of advance directives into TPOPP/POLST orders, and implementation plans.

1. “Drinking and Drugging in Long-Term Care”

Substance use disorder in older adults has been under-identified for decades. In addition, the aging of the baby boomer generation has changed attitudes towards rates of substance use disorder. Senior Care facilities are accepting more residents who have problems with alcohol or drug use. Although facilities are not expected to be able to cure substance use disorders, updated guidelines and F-Tags have increased the responsibility of the facilities in terms of treating these residents. Therefore, it is important for all staff to understand assessment and treatment approaches for substance use disorder. Discussion will include attitudes toward older adults with substance use disorder as well as statistics and information regarding substance use in older adults. Updated guidance regarding the F-Tags related to substance use will be explained. In addition, this course will focus on training health care providers to assess substance use in facilities.

2. “Documentation Do’s and Don’ts and an Overview of the IDR Process: A Guide for LTC Facilities”

Part One of this presentation will cover best practices and common errors with respect to medical record documentation for post-acute care providers (skilled nursing and assisted living facilities). It will also touch on responding to medical records requests and subpoenas. Part Two will provide an overview of the IDR process in Missouri, including options available to facilities for pursuing IDR after receiving a survey deficiency, timeframes in which to request and pursue IDR, and different IDR strategies, including whether or not to involve counsel in the IDR process, which deficiencies to contest, and common arguments to consider raising.

This session will explore how the AHCA/NCAL Quality Award criteria can be leveraged not just as an organizational improvement tool, but as a powerful framework for enhancing resident-centered care in long-term care settings. 4. “The Leader’s Role in Skilled & Assisted Living Occupancy”

Traditional marketing and outreach strategies that once drove SNF admissions and ALF move-ins aren’t as effective in today’s senior care marketplace. Across the country, the pressure for SNFs to meet payer and quality expectations is enormous. Increased resident acuity has altered the competitive landscape in both SNFs and ALFs–in managing the demand for higher clinical capabilities and finding staff to support it. For both SNFs and ALFs, a retrospective look-back at the number of admissions/move-ins is unproductive. A lost admission or move-in and lost revenue is nearly impossible to make up for in any given month. Senior care providers need to be proactive in creating a steady stream of inquiries/referrals by monitoring 3 data sets that enable more nimble strategies. Frequently calculating and monitoring these benchmarks enables community leadership and their marketing team to recognize (and act) on any given day in the month potential downturns in admissions/move ins–before they occur. Lastly, a greater focus on describing the community’s success in respect to outcomes, quality, and lengths of stay is imperative. This session provides the outline for an executive level report for use with MCO leadership. Following this session, participants will be able to:
- Redirect their current focus on to-date admissions/move-ins each month to 3 more valuable benchmarks impacting strategy
- Calculate the 3 benchmarks that predict rise/fall in occupancy
- Respond proactively when key benchmarks deviate from expectations
- Create the compelling argument to be chosen (by health care organizations and families)

1. “How Does the Discharge Function Score Function?”

The SNF QRP required discharge function score measure is the GG replacement measure for the section G based short stay improvement in function. Since January 2024, CMS has been calculating the new discharge function score on IQIES reports. The new measure began to affect 5-star ratings in January 2025 and will affect the Value-Based Purchasing incentive multiplier in October 2026. This presentation will help providers to become familiar with how this quality measure is calculated, how it will impact each SNF quality program, and how to use the results for quality improvement. Objectives are: Identify the differences between the GG-based discharge function score and the G-based improvement in function score; Understand the technical specifications of the discharge function measure; Become familiar with the expected discharge function score; Interpret how this measure impacts SNF quality measurement programs; Apply the results to a facility-level quality improvement effort.

1. “SNF VBP QM Scoring Expansion. Ready, Set, Go!”

With the SNF Value Based Purchasing incentive multiplier set to be impacted by 4 quality measures in FY 2026 and a total of 8 measures by FY 2027, CMS has revised the scoring methodology to accommodate the additional measures. CMS has also posted early look feedback reports to IQIES to prepare providers for this change. SNFs should become familiar with the scoring methodology changes so that they will be better able to understand their VBP incentive multiplier as these changes take effect. This session will also preview October 2026 when 4 more measures will be added. Objectives are: 1. Identify the expanded SNF VBP measures; 2. Recognize the components of the incentive multiplier scoring methodology; 3. Differentiate between the improvement and achievement score; 4. Understand the score normalization methodology; 5. Consider Health Equity Initiatives within the VBO and QRP programs.
