To obtain a copy of any license or photo I.D., please submit the following in a written request to us:

- Name as you were certified
- Social Security Number
- Picture that can be cut up
- Date of Birth
- Certificate number if known

The cost for a copy is $5.00 for a certificate and/or $5.00 for a picture I.D. Please send this in the form of a check, money order, or MasterCard or Visa information. We DO NOT bill for copies. We also CAN NOT fax a copy of the certificate or I.D.
